Records we hold

We keep records under the Health Professions Act 56 of 1974, National Health Act 61 of 2003, Protection of Personal Information Act 4 of 2013, Income Tax Act 58 of 1962 and Tax Administration Act 28 of 2011 and, where they apply, VAT, labour and company laws.

The content of this website is available without a request.

Personal information

• We hold information about patients and their parents or guardians (identity, contact, health and billing details), referring health professionals, suppliers and staff.
• We use it to provide healthcare, manage bookings and invoices, and keep the records the law requires.
• We share it only with health professionals involved in a patient's care, laboratories, our service providers (Cliniko for bookings and records, Google for email) and authorities where a law requires it.
• Cliniko stores records on servers in Australia. Google stores email on servers outside South Africa.
• Only staff who need the information can access it. Cliniko encrypts the data and backs it up daily.

How to request a record

1. Complete Form 2 from the Information Regulator's website and send it to the Information Officer.
2. Pay the request fee of R140. Any access fee follows the Regulator's fee schedule.
3. We decide within 30 days. We may extend this once by up to 30 days.
4. If you disagree with our decision, you may complain to the Information Regulator or apply to court within 180 days.

Patients can ask reception for a copy of their own file.
